- 2004 – created by GPs in West Yorkshire from nine co-operatives
- 2004 – wins 15 contracts to provide out of hours services across West Yorkshire
- 2006 – British Association of Medical Managers Team of the Year
- 2006 – expands to cover South Yorkshire and Lincolnshire
- 2008 – wins a contract for the country’s first GP-led equitable access centre in Bradford
- 2009 – expands primary care business
- 2012 – launches partnership with Yorkshire Ambulance Service NHS Trust to deliver NHS111 and urgent care services in West Yorkshire and the Craven District of North Yorkshire
- 2012 – develops a new clinical management model of healthcare using ‘Telehealth’ technology for a pilot service in Leeds
- 2013 – selected by Skills for Health as a national pilot site for a role modelling project to create a new support role specification for health care providers
